Supporting patients moving from hospital to home care

For many people with complex care needs, leaving the supportive environment of a hospital can be tough.

Having the right support after a hospital stay ensures a smooth transition for your loved one, allowing them to maximise their independence.

Whether someone requires short-term or long-term support following a hospital stay, we can help develop the right care plan to fit their needs.

Setting up care

Planning for aftercare begins at the hospital. During your loved one’s hospital stay, the staff will work alongside them and their family to establish what support is needed. This will include:

  • How the doctor will decide that the patient will be discharged
  • When this discharge is expected to happen
  • Where the patient will go, including considering community hospitals or care homes
  • Transport and other arrangements
  • Support and care that may be required.

Once this is established, then care can be discussed. There is often free, short-term care available in the form of aftercare. However, you can choose to source your own care providers for your loved one.

At Synergy, we work alongside your loved one’s healthcare professionals to ensure that all their needs are met outside the hospital. This includes establishing a bespoke care plan catering to your loved one’s needs.

When you are discharged

A discharge co-ordinator will work with a doctor to decide when your loved one will be sent home. They will liaise with our nurses to ensure they are prepared to deliver the care you need at home.

In some instances, your loved one may require specific transport options. This can be arranged with the hospital in advance so they can get home safely and comfortably.

Once care at home begins, our nurses will continue to liaise with your loved one’s healthcare team to deliver the care that they need. This can include support with:

  • Mobility
  • Ventilation and airway management
  • Cooking and cleaning
  • Personal hygiene
  • Medication management
  • Communication accommodations
  • Attending follow-up hospital appointments
